This site includes forms designed by the Pennsylvania Department of Education to ensure that students with disabilities receive a free, appropriate, public education. Forms are available for the Infant-Toddler/Early Intervention, Preschool/Early Intervention, and School-age programs, and include both annotated and non-annotated versions. Forms are available as Word and pdf documents.

INVITATION TO PARTICIPATE IN THE IEP TEAM MEETING OR OTHER MEETING - School Age
Rivisto Maggio 2015 - Questo nuovo invito a Partecipare all'incontro della squadra IEP o ad altri incontri (con piè di pagina che indica "Revisioni Maggio 2015") sostituisce l'invito attuale. Questo modulo rivisto aggiunge scopi supplementari per un incontro della squadra IEP, elimina il bisogno di utilizzare due inviti separati per partecipare all’Incontro della Squadra IEP o ad altri incontri per la pianificazione/servizi di transizione e completa le spiegazioni di pianificazione e servizi di transizione, inserendo una dichiarazione nella quale l'agenzia locale dell'educazione (LEA) inviterá lo studente se ha 14 anni o di piú. Questo modulo entra in vigore a partire dal 30 giugno 2015. Disponibile la Versione annotata.

PARENTAL CONSENT TO EXCUSE MEMBERS FROM ATTENDING THE IEP TEAM MEETING - School Age
Ce formulaire prend effet le 1er juillet 2008 et reflète les nouvelles dispositions des chapitres révisés 14 et 711. Cette nouvelle invitation (avec un pied de page indiquant «Révisions de juillet 2008») remplace l'invitation en cours. Cette invitation ne contient plus la demande de consentement des parents pour exempter les membres de l'équipe IEP d'assister à la réunion de l'équipe IEP. Cette demande est maintenant faite avec un formulaire distinct, «Consentement parental pour exempter les membres d'assister à la réunion de l'équipe IEP». Version annotée disponible.

NOTICE OF RECOMMENDED EDUCATIONAL PLACEMENT/PRIOR WRITTEN NOTICE (NOREP/PWN)
