Secondary Transition Outcomes: My Plan 4 Succes

Indicator 14 addresses the extent to which students are engaged in education and/or employment after high school. Each school district and charter school is required to survey all student leavers who had IEPs (graduates, dropouts and students that reached maximum age) once over a five-year cycle.

Indicator 14 data is collected over a two-year process. During the first year, exit survey data collected for all leavers. In year 2, the post survey data is collected one year after exit specific to the percent of students:

A. Enrolled in higher education within one year of leaving high school.

B. Enrolled in higher education or competitively employed within one year of leaving high school.

C. Enrolled in higher education or in some other postsecondary education or training program; or competitively employed or in some other employment within one year of leaving high school. 

My Plan for Success (MP4S) Exit Process Overview and Procedural Training

This training webinar and supporting materials will provide participants with an understanding of the My Plan for Success (MP4S) and the requirements for implementation. The purpose of MP4S is to determine, for the state as a whole, the extent to which students are achieving transition outcomes as stated in their Individualized Education Programs (IEPs). This is a required training for LEAs.

Accountability requirements under Individuals with Disabilities Education Act 2004 (IDEA) regarding secondary transition mandate each state to determine, for the state as a whole, the extent to which students are achieving transition outcomes as stated in their IEPs (Indicator 14). To meet this federal reporting requirement, once over a five-year period, each school district is required to survey all student leavers who had IEPs (graduates, dropouts, and students who age out). This session will provide target LEA´s, special education administrators, and IU transition consultants information regarding the My Plan for Success (MP4S) and the requirements for completing the MP4S Exit Process.
