Bridging Connections 4 Employment: Regional Planning for the Future

This interactive session will bring together Connecting for Employment regional teams to collaboratively assess the current landscape of agency functioning, identify barriers and successes, and develop actionable plans for the 2025-26 school year. Using the Charting the LifeCourse (CtLC) framework, teams will engage in guided discussions and problem-solving activities to strengthen regional coordination and enhance transition outcomes for youth with disabilities. Participants will also take part in state community mapping work aligned with the Pathways to Partnership (P2P) grant deliverables to support effective interagency collaboration.
Objectives
By the end of this session, participants will:
- Analyze the current landscape of agency functioning in their region, sharing successes and identifying barriers.
- Apply best practices and structured planning tools, such as the Life Trajectory, Integrated Supports Star, and CtLC Goal Attainment, to inform regional action plans.
- Develop a region-specific strategy for improving transition services, leveraging community resources and partnerships.
- Engage in guided community mapping activities to align efforts with the P2P grant deliverables and enhance cross-agency collaboration.
Target Audience
Connecting for Employment Regional Teams, including educators, agency representatives, and stakeholders involved in transition planning.
