During this event, participants will learn about the foundations of Transition Discoveries (TD), including the framework, the purpose, the multi-stage implementation model, team structure and timelines. Building upon this foundational knowledge, participants will learn about the purpose, process, related tools and essential strategies for supporting LEAs in implementing Stage 1 of Transition Discoveries. Participants will explore relevant TD Tools for Stage 1, including the TD Quality Indicator Survey (TDQI), survey process tracker, and monthly implementation update (MIU) tool. Participants will experience using these tools as if they were from the LEA (Local Education Agencies) using case data from a fictitious LEA. Participants will also practice coaching LEAs through using these tools given case study examples. Case study activities will also include working with LEAs through challenges with establishing a Transition Change Agent Team (T-CAT), gaining administrative support, identifying a representative sample, distributing the TDQI, managing parent permission forms, tracking survey progress and finding creative ways to fund incentives for youth, families and stakeholders to complete the TDQI that adhere to grant and federal spending requirements.
Objectives
After this event, participants will be able to:
(1) describe the components of the Transition Discoveries Framework
(2) Describe the purpose of implementing Transition Discoveries
(3) Describe the components of the four stage Transition Discoveries implementation model (4) Describe the structure and function of the LEA’s Transition Change Agent Team (T-CAT)
(5) Support LEAs in using the Monthly Implementation Update (MIU) to plan during Stage 1, (6) Coach LEAs through Stage 1 activities, including: a) identifying a representative sample, b) distributing the TDQI, c) managing parent permission for student TDQI survey completion, d) tracking survey progress, e) developing incentives for survey participants that adhere to grant and federal spending requirements and f) empowering youth and families to be actively engaged in Stage 1 of TD implementation.
