During Year 2 of Transition Discoveries: Pathways to Competitive Integrated Employment, the LEA’s Team Champion and when feasible, additional members of the LEA’s T-CAT, will complete a self-guided module within Schoology. The module teaches them how to further develop their community action vision board into a comprehensive implementation plan with detailed action sequences for identified goals and their responsible parties. The T-CAT learns core concepts such as the implementation setting/context, its potential factors and how to both identify and address them proactively. Finally, the module teaches them how to systematically implement their plan, and to sustain those efforts at a district-wide / community-wide level within and across relevant systems (e.g., education, vocational rehab, etc.).
Objectives
At the end of this training, T-CAT members will be able to:
Develop a comprehensive plan, based upon the community’s vision board, to address short- and long-term goals aligned with one or more of their priority areas as identified by their TD Data
Identify an implementation context and implement strategies to address factors within it that have influence on their action plan
Implement strategies to sustain their action plan’s implementation after the funding period ends

LEAs who have been selected to participate in the Transition Discoveries: Designing Pathways to Competitive Integrated Employment Grant.
