Charting the LifeCourse - High Expectations for Transition

This session will offer the audience an in-depth overview of the history and genesis of Charting the LifeCourse from a national and Pennsylvania perspective. Additionally, audience members will learn about each of the elements of the Charting the LifeCourse framework and principles, as well as the foundational Life Course Tools. Audience members will have the opportunity, during the final portion of the session, to apply what they learned about the framework and principles by using the LifeCourse Tools on themselves in a small-group fashion, thereby increasing their sense of self-efficacy in utilizing these tools within their personal and professional environments after the session ends.
Objectives
Juniata College to provide a sample overview of community supports for students after high school.
Describe the National Community of Practice for Supporting Families Throughout Lifespan
Discus the LifeCourse Principles and Framework
List three benefits of the LifeCourse framework and tools to be used in life stage transitions
Apply the framework and principles to personal and professional environments
Target Audience
Essential participants: transition coordinators
Other interested parties: school counselors, providers, administrative entities, supports coordinators, managed care organizations, community partners
