Daily Evaluation Surveys

 Credit Surveys and Session Verification

 Daily surveys will be available on this page after the last session of the day.

 Each day's survey will need to be completed by Wednesday, November 22, 2023 at 4:00 pm in order to receive continuing   education credits.
 
 If you are seeking Act 48, ASHA, Psych, or ACVREP you must complete these surveys by the November 22 deadline.

 For ASHA credits, please be sure to complete all questions on the daily surveys and submit them by the deadline. Also note   that all time is cumulative for sessions eligible for ASHA credit, but it is rounded down to the lowest half-hour.
 
 If you are seeking other credits (e.g., OT, PT), you must choose "instructional hours" when you register, and then complete   the daily evaluation surveys. By completing the daily evaluation surveys, you will then be able to download a certificate of   attendance that you can then use to submit on your own to the respective organizations for other credits (e.g., OT, PT).
