Transition assessments are crucial for empowering students to learn about themselves so they can take an active role in their career development and post-secondary goal planning process. For students currently receiving special education services or related services, as a part of their transition planning under IDEA, outside agency involvement should be considered every year after the students reach the age of 16, or earlier if determined appropriate by the IEP team. The school must invite a representative of any participating agency that is likely to be responsible for providing or paying for transition services (IDEA Sec. 300.321 (b) (3). This series will address how various agency partners use assessments so that Individualized Education Plan (IEP) team members can consider which agency is best positioned to conduct a specific assessment to avoid duplication of services, and determine how information will be shared between agencies and the process to obtain required parental consent.
