Transition

Students with disabilities experience several transitions throughout their education. At three years of age, activities occur to transition them from the infant/toddler program to preschool. During the year before the child is eligible for kindergarten or first grade, transition activities take place to plan a successful transition to the local district's K-12 program. A final transition occurs as the student moves to the vocational world and the community.

Early Intervention Transition
Early Intervention Transition addresses planning for a child who receives special education services to move from the infant/toddler programs to Preschool programs and services. Another transition occurs as the student moves from Preschool into the K-12 school district program.

Secondary Transition
Secondary Transition is the process of preparing students for life after they leave high school, including successful participation in post-secondary education/training, employment, and community living. These areas are often referred to as "post school outcomes" and are the driving force behind Individualized Education Programs (IEPs) written for students with disabilities while in high school. Goals for post school outcomes are based on career education and assessment provided by school districts in middle school and continued through the high school years. Instructional programs are then provided based on the student's career goals and assessment, as well as the outcomes that have been determined by the IEP team. Transition planning helps students successfully cross the bridge between school programs and the opportunities of adult life.
