Special Education Forms

The special education forms are designed to ensure that all students with disabilities attending charter schools have available to them a free appropriate public education, which is designed to enable the students to participate fully and independently in the community, including preparation for employment or higher education. The forms include all items required under state and federal regulations.

The forms have been updated to reflect new Chapter 711 regulations, which went into effect on JUly 1, 2008

Material Description:
The primary purpose of the Communication Plan is to establish discussion among all IEP team members who review the student's needs based on communication skills and access. The result of this discussion should be used to identify the student's present educational levels as well as to complete other sections of the IEP including appropriate specially designed instruction and IEP goals.
