The primary purpose of the Communication Plan is to establish discussion among all IEP team members who review the student's needs based on communication skills and access. The result of this discussion should be used to identify the student's present educational levels as well as to complete other sections of the IEP including appropriate specially designed instruction and IEP goals.

This annotated document describes the contents of the Evaluation Report, which is completed to document the process in which an evaluation team, defined in Federal and State regulations as a group of qualified professionals and the parent of the student, determine if the student is a student with a disability and in need of special education services. This updated document (with footer indicating "October 2009 Revisions") contains the revisions made to the Participation in State and Local Assessments pages of the IEP, which now includes the PSSA-Modified in Math as an option for eligible students. This annotated document describes the contents of the Individualized Education Program (IEP), which is developed by an IEP team for each student who is determined to be a student with a disability and eligible for special education supports and services.

The recent supplemental regulations to 34 CFR Part 300, effective December 31, 2008, require that Local Education Agencies provide parents with prior written notice upon parental revocation of consent for the continued provision of special education and related services. This annotated form, revised December 31, 2008, describes how to use the current NOREP/PWN to provide this notice to the parents.
