INDIVIDUALIZED EDUCATION PROGRAM (IEP) - School Age These forms are based on changes to the Individuals with Disabilities Education Act (IDEA) as amended in 2004. This form is effective beginning April 1, 2006. This new IEP form (with a footer indicating "PDE Revised March 2006") replaces the current IEP. Changes were made to Sections VII and VIII to meet requirements of the Gaskin Settlement Agreement. Please refer to PennLink dated 3/30/06 for additional information.
