Interdisciplinary Transition Team Initiative

Transition planning for young adults is a vital process for the student, family and team members in which to engage. It is doubly important for students with combined vision and hearing loss as many players must come to the table to assist in making a smooth and satisfactory transition from school to adult life. Planning must begin as early as possible in order to have a person-centered, comprehensive plan. The Interdisciplinary Transition Team Initiative (ITTI)is a year long focused intervention to assist a team with the complexities of planning for and with a student with deaf blindness.
